AMS Solutions manages the revenue cycle from accurate charge entry through payment posting and follow-up, helping your team reduce preventable denials and focus on patient care.

Urgent care revenue cycles move quickly, but small front-end or coding errors can delay payment. AMS Solutions helps centers address the issues that most often slow reimbursement:

  • Eligibility and demographic errors caused by walk-in visits and incomplete information
  • Coding complexity across varied services, tests, treatments, and payer requirements
  • Payer-specific rules, documentation gaps, and preventable claim denials
  • Delayed follow-up, appeals, and aging accounts receivable

Why Urgent Care Billing Is Different

Urgent care sits between a primary care office and an emergency department, and the billing reflects that position. A family practice bills a known panel of established patients against a contracted fee schedule. An urgent care center bills whoever walks through the door — a commercial member, a Medicaid enrollee, a self-pay patient, an employee sent over by a local employer, a traveler from out of state — sometimes inside the same hour.

Several practical differences follow from that:

  • Almost every patient is new. There is no established panel to fall back on. New-versus-established determinations, demographic capture and insurance verification all happen at the front desk in real time, under volume pressure.
  • The payer mix changes daily. A center can see commercial, Medicare, Medicaid, workers’ compensation, auto liability and self-pay in a single shift — each with different timely-filing rules, authorization expectations and appeal paths.
  • One visit spans several service categories. An encounter may include an office evaluation and management service, a point-of-care test, an injection, a splint, or a laceration repair. Each carries its own documentation requirement and its own bundling rules against the others.
  • Evenings and weekends are the core of the business. After-hours and holiday coverage is the product, not an exception, and payers differ sharply on whether they recognize after-hours reporting on top of the visit.
  • Some payers want urgent-care-specific reporting. Certain commercial plans contract on a global fee and expect HCPCS code S9083 (global fee, urgent care centers) instead of itemized coding. Others recognize S9088 (services provided in an urgent care center, reported in addition to the code for the service) alongside the underlying visit. Which applies — or whether either applies at all — is contract-specific. Billing a global fee to a payer that expects itemized claims, or itemizing for a payer on a global arrangement, produces denials in both directions.

A billing team that treats an urgent care center like a small clinic will get the front end wrong and miss the payer-specific reporting rules entirely.

The Denial Patterns We See Most Often in Urgent Care

Urgent care denials cluster. The same handful of root causes accounts for most of the rework we find when we review a center’s aging:

  • New versus established patient errors. The determination turns on whether the patient received a face-to-face service from a provider of the same specialty and subspecialty in the same group inside the prior three years. Front-desk turnover, multiple locations under one tax ID and rotating clinicians all make this easy to get wrong.
  • Evaluation and management level selection. Under the current office and outpatient framework, level is driven by medical decision making or by total time on the date of the encounter, not by history and exam bullet counts. Providers documenting from old habits tend to under-code straightforward visits and leave complex ones unsupported.
  • Missing modifier 25. When a significant, separately identifiable evaluation and management service is furnished on the same day as a minor procedure, modifier 25 belongs on the evaluation and management line, and the note has to support the visit as distinct from the procedure on its own terms. Missing or unsupported modifier 25 is one of the most common urgent care denials we work.
  • Unbundled point-of-care testing. Rapid strep, influenza, respiratory panels and urinalysis are routine in urgent care and routinely denied — the test bundles into another service on the claim, the waived-test modifier a payer requires is missing, or the diagnosis does not support medical necessity for that test.
  • Laceration repair and fracture care documentation gaps. Repair coding depends on wound length, anatomic site and whether closure was simple, intermediate or complex. Fracture and dislocation care depends on whether the provider assumed restorative treatment or only stabilized and referred out. When the note omits the measurement or leaves treatment intent ambiguous, the claim downcodes or denies.
  • Workers’ compensation claims sent to the wrong payer. An occupational injury billed to the patient’s health plan gets denied, sits in aging, then runs into the compensation carrier’s filing deadline on the rebill. Catching employer-related visits at registration prevents the whole sequence.
  • Credentialing gaps for new and rotating providers. Urgent care leans on locums, part-time clinicians and advanced practice providers. When a clinician sees patients before payer enrollment is effective, claims deny or have to be held. Our provider credentialing team tracks effective dates so the schedule and the enrollment calendar stay aligned.

How AMS Runs Urgent Care Billing

Our workflow is built around the pace of a walk-in clinic rather than a scheduled practice:

  • Eligibility checks at check-in. Coverage, plan type, copay and whether the visit is employer-related get verified while the patient is still in the building, so corrections happen before the claim is built.
  • Charge capture within 24 hours of the encounter. Our workflow target is to have the previous day’s encounters coded and submitted the next business day. In a high-volume walk-in setting, charges that sit for a week become charges nobody can reconstruct.
  • Coding review by AAPC-certified coders. Encounters are reviewed against the documentation before submission, with attention to level support, modifier use and the reporting rules in that center’s payer contracts.
  • Denial work queues organized by payer. Denials are grouped by carrier and reason code rather than worked in date order, because the fix for a given payer is usually the same fix repeated — which also shows what to correct upstream at registration or in the note template.
  • Credentialing and payer enrollment for rotating clinicians. Enrollment, revalidation and roster maintenance are tracked continuously rather than handled as one-time paperwork.
  • Monthly reporting on metrics that move. Days in accounts receivable, first-pass acceptance rate, denial rate by reason, and aging by payer bucket — reported consistently month over month so the conversation is about causes rather than anecdotes.

Occupational Medicine and Workers’ Compensation

Most urgent care centers carry some occupational medicine volume, and it bills nothing like the rest of the schedule. Work injuries, pre-employment physicals, drug screens and return-to-work evaluations run through a separate path.

What goes wrong is usually structural rather than clinical. Employer accounts have to be set up so services billed directly to a company stay separate from services billed to a workers’ compensation carrier. Compensation reimbursement follows a state fee schedule rather than a negotiated commercial rate, so posting and adjustment logic differs from the rest of the book. First-report-of-injury filings carry their own forms and deadlines, and a late report can hold payment on an otherwise clean claim. Authorization for follow-up treatment runs through an adjuster, not a health plan portal. We run these as distinct billing paths with their own follow-up cadence.

Telehealth and After-Hours Visits

Many urgent care centers added a virtual front door and kept it. The rules behind it have moved repeatedly, and what a payer covered two years ago is not a safe assumption. Medicare telehealth flexibilities currently run through 31 December 2027; commercial and Medicaid coverage varies by plan and by state, and place-of-service and modifier expectations vary with it.

After-hours reporting works the same way. Some payers recognize an add-on for services furnished outside posted business hours or on holidays; many fold it into the contracted visit rate and deny it on sight. Knowing which contracts recognize it — and not billing it where they do not — is cheaper than absorbing the denials.

What to Bring to a Free Urgent Care Billing Review

A useful review works from real data rather than a general conversation. If you can pull the following, we can tell you where the money is sitting:

  • The last 90 days of denials, grouped by reason code and payer
  • Accounts receivable aging by payer and by bucket
  • A current provider roster with enrollment status and effective dates for each payer
  • Your contracted fee schedules, including any global fee or urgent-care-specific arrangements
  • Your top 25 CPT codes by volume for the same period
  • A short list of the payers your staff finds hardest to collect from, and why

From that we can usually separate which denials are front-end, which are coding and which are enrollment — three different problems that look identical on an aging report.
